Implementing Evidence-Informed Interventions with Black MSM to Improve Access, Engagement/Reengagement, and Health Outcomes
This peer-led workshop will explore the implementation of evidence-informed interventions with black men who have sex with men (MSM) through Project CONNECT at AIDS Taskforce of Greater Cleveland, Tailored Motivational Interviewing at the University of Mississippi, and TXTXT at the SUNY-Downstate HEAT Program in Brooklyn. Attendees will learn the core elements of these interventions to improve engagement and viral suppression.

Positive Love: Improving Health Outcomes of Young, Black MSM with HIV in the Deep South
The presentation will cover the efficacy of +Love, a five-pronged intervention in New Orleans, involving a dedicated case manager, therapist, peer support, ride service, and text-based alert program that tracks clients' basic needs, mood, and provides medication reminders, in improving the healthcare and treatment satisfaction of young, black men who have sex with men (MSM) with HIV.

National Implementation of a Text-Messaging Intervention to Improve Antiretroviral Adherence among Youth with HIV
This workshop will explore two sites' (Brooklyn and Detroit) experience with the implementation of an evidence-informed intervention for black men who have sex with men (MSM) with HIV. The project seeks to improve HIV outcomes through the use of a culturally tailored intervention for Black MSM, the Text Messaging Intervention to Improve Antiretroviral Adherence among youth with HIV (TXTXT).

Developing Systems to Collect, Report, & Use Data to Optimize CHW Initiatives in the South (16076)
Through the Southern Initiative, CAI supported four Part A agencies in the South in integrating a Community Health Worker (CHW) model to address disparities in HIV outcomes. Health Information Exchange and Peer Navigation: A Model to Engage Vulnerable Populations
Presenters will describe a New York City collaborative pilot project utilizing Regional Health Information Organizations (RHIOs) to locate lost-to-care people with HIV. Lost-to-care encounter alerts provide community-based organizations with actionable, real-time data to supplement their ‘classic' care engagement efforts. The model offers a potentially scalable, cost-effective strategy for patient re-engagement efforts on a population level.

Utilizing an HIV Clinical Pharmacist to Improve Patient Outcomes
This presentation will review the many roles a clinical pharmacist can play in improving patient care, such as providing medication education, conducting adherence counseling sessions, facilitating transitions in care, providing comprehensive medication chart reviews, and aiding with the formation of complex treatment regimens. The programs are Healthy Divas, and Transgender Women Engagement and Entry to Care Project (T.W.E.E.T.). Community Health Workers Improve Viral Load Suppression in Non-Treatment Adherent Populations to End the Epidemic
Community Health Workers (CHWs) were added to the care team to work with clients with poor treatment adherence and not viral load suppression (VLS). Clients benefited from CHW services that contributed to VLS in over 50% of the clients assigned to them. CHWs are vital in ending the epidemic in challenging client populations
