This presentation will review the many roles a clinical pharmacist can play in improving patient care, such as providing medication education, conducting adherence counseling sessions, facilitating transitions in care, providing comprehensive medication chart reviews, and aiding with the formation of complex treatment regimens. Pharmacists' involvement in pre-exposure prophylaxis will also be reviewed.

Session 2 of the Leadership Institute focuses on stigma and intersectionality. Participants will explore different forms and manifestations of stigma, the impact of stigma on health outcomes, and the tenets of intersectionality. Quality improvement is dependent on a strong system of data collection, management, and analysis. The Learners Education and Practice Portal (LEAPP) developed by the MidAtlantic AIDS Education and Training Center (AETC) at the University of Pittsburgh has an integral role in providing AETCs with quality improvement opportunities through the Six Sigma model of quality improvement.

The New York City Department of Health and Mental Hygiene designed an HIV self-management protocol for the Ryan White Part A care coordination program. Through the protocol, staff and patients systematically identify and address patient strengths and challenges, focusing activities on building patients' capacity to manage their care.

People with HIV 50 years and older is a rapidly increasing population with unique needs. The presentation summarizes a population-based approach to assess those needs and to respond with training and other resources. Presenters will describe the implementation of the approach by the New York City EMA in partnership with the Northeast/Caribbean AETC.

This presentation focuses on peer-based training for engaging consumer voices in HIV providers' quality improvement efforts. Community-based, peer-led efforts impact HIV care program effectiveness. Peer-led training concentrating on client advocacy and involvement in quality improvement is another method for promoting intentional collaboration between HIV service programs and people with HIV.

This workshop will explore the implementation of peer-led, evidence-informed interventions for transgender women with HIV. The programs are Healthy Divas, and Transgender Women Engagement and Entry to Care Project (T.W.E.E.T.). Attendees will learn the core elements of these interventions and how they improve engagement in care, treatment adherence, and viral suppression in Alabama and Michigan.

Community Health Workers (CHWs) were added to the care team to work with clients with poor treatment adherence and not viral load suppression (VLS). Clients benefited from CHW services that contributed to VLS in over 50% of the clients assigned to them. CHWs are vital in ending the epidemic in challenging client populations

Motivational interviewing techniques are applied in the program evaluation context, using examples from Ryan White programs. Using motivational interviewing, the evaluator leads the dance in the desired direction while incorporating stakeholder input.

The game was developed to address the common scenarios that occur which cause stress and confusion in the battle to weigh client needs against agency ethos and program requirements. The game promotes motivational interviewing skills for problem-solving with consumers and coworkers alike.

No-show rate is a strong and independent predictor of patients falling out of care at the partnership that cares for 1,700 people with HIV in Philadelphia. A pilot study was created to address every no-show event in an effort to prevent patients from falling out of care.
