This workshop will describe how the Los Angeles Area AETC developed an inter-professional practice (IPP) program with the Charles Drew NP program, USC’s School of Pharmacy, UCLA’s School of Medicine, and the West LA’s psychiatric residency program -- along with five Ryan White clinics -- to help develop the next generation of HIV teams.

This workshop will describe how changes to an electronic health record (EHR) can capture programmatic outcomes data to evaluate program effectiveness, the benefits of developing custom tools within EHRs, how these EHR tools were developed, and how they have been integrated into program and evaluation work processes.

This session will give a firsthand perspective on aging with HIV, provide an overview of RWHAP aging data, summarize medical conditions and psychosocial needs facing aging clients, and review HRSA initiatives.

As HIV elimination activities increase, jurisdictions can address HIV/Hepatitis C (HCV) co-infection by enhancing HCV screening and treatment data collection. This session will discuss NASTAD's work with North Carolina to incorporate HCV data in CAREWare and electronic health records (EHRs), as well as recommendations for integrating HCV services into the HIV care infrastructure.

The session will discuss lessons learned from the Rhode Island's path to the AIDS Drugs Assistance Program (ADAP) automation through the implementation of the Lifia Portal. People with HIV 50 years and older is a rapidly increasing population with unique needs. The presentation summarizes a population-based approach to assess those needs and to respond with training and other resources. Presenters will describe the implementation of the approach by the New York City EMA in partnership with the Northeast/Caribbean AETC.

This session will help recipient and subrecipient sites to successfully connect their CAREWare and other electronic health information systems with external sources of data to report and monitor HIV care and treatment outcomes. This session will provide practical examples of successful data integration and highlight ongoing issues and costs in long-term management and maintenance.
