This interactive workshop discusses the process, facilitators, barriers, and lessons learned while conducting two End the Epidemic, NIMH-funded implementation science planning grants awarded to the AIDS Research Center at Columbia University in partnership with the Northeast/Caribbean AETC and several Departments of Health, Ryan White recipients, and community stakeholders.

This session will outline establishment of community partnerships and implementation of housing, nutrition, and substance use treatment programs. It will detail contract development between the presenters’ clinic and community partners and outline how, following policy clarification notice (PCN) 16-02 guidance on allowable uses of program income, community partners are reimbursed in support of these programs.

The Northeast/Caribbean AIDS Education and Training Center HIV Health Information Exchange (HIE) Initiative aims to empower HIV care networks to utilize the potential of emerging data exchange models toward improving HIV outcomes and ending the epidemic. Unstable housing affects the ability of people with HIV to access, adhere to, and be retained in HIV medical care across the HIV care continuum. This presentation will examine adaptions made to housing and case management services in a Ryan White program to accommodate homeless and near-homeless people with HIV.

This session will present strategies to improve viral suppression in clients experiencing unstable housing and homelessness. This includes both a recipient serving homeless clients and a RWHAP AIDS Education and Training Center that trains clinicians on strategies to serve clients experiencing homelessness.
