Bottom-Up Project
The Bottom-Up Project is a multi-organizational initiative focused on leveraging health information exchange data and peer navigation. Using real-time clinical data, in combination with linkage to HIV care and social services, the Bottom-Up Project locates and reengages people with HIV who are not currently in medical care and are not virally suppressed. Creating a Culture of Trauma-Informed Care
Lehigh Valley Health Network Comprehensive Health Services implemented a trauma-informed approach to care delivery, including training staff on how trauma can affect people’s health and how microaggressions in healthcare environments can potentially trigger trauma responses. Providing HIV Services to People Who Are Incarcerated
The Maricopa Jail Project was implemented by five jails to decrease the wait time between incarceration and/or diagnosis to the start of treatment, and to better support clients to reach viral suppression. Maricopa hired a nurse practitioner to manage access and case manage across the jail system. Peer Engagement to Improve Linkage to Care and Improve Retention for Youth, Women, Infants and Children
University Health System’s peer engagement initiative employs peers, alongside Patient Navigators, to provide support and improve linkage and retention to care for youth, women, infants, and children. Project CAATCH: A Quality Improvement Initiative

Outcomes of a Transitional Care Coordination Intervention Linking People with HIV in Jails to Community HIV Care and Services
Three sites were funded in 2016 as part of Special Projects of National Significance Dissemination of Evidence-Informed Interventions initiative to implement an evidence-informed intervention designed to link people with HIV in jail to community HIV care using a transitional care coordinator. Presenters will discuss the barriers and facilitators related to linking to care and achieving and maintaining viral suppression post-release.
