Telepsychiatry has proven to provide better access and higher-quality care to patients who need psychiatric care as well as for those who have varying circumstances that make it difficult to engage in this service. Vivent Health has successfully integrated telepsychiatry within its medical home model. With two different service delivery methods, this presentation will look at the benefits of telepsychiatry for people with HIV, as well as the unique delivery methods Vivent Health provides in Wisconsin and Colorado.

This session will present strategies to improve viral suppression in clients experiencing unstable housing and homelessness. This includes both a recipient serving homeless clients and a RWHAP AIDS Education and Training Center that trains clinicians on strategies to serve clients experiencing homelessness. In this workshop, two sites in HRSA's Improving Health Outcomes through the Coordination of Supportive Employment and Housing Services Initiative will describe how they work with their Part A Planning Councils and the Department of Housing and Urban Development's (HUD) Coordinated Entry System to identify and obtain permanent housing for people with HIV who are unstably housed.

Boston Healthcare for the Homeless Program uses an innovative care model, designed from the margins, to meet the complex needs of people living at the intersections of HIV, homelessness, substance use disorder, and incarceration. Such models hold promise for closing HIV care and prevention equity gaps for this hyper-vulnerable group.

Establishing an opt-out HIV screening program in the emergency department has almost quadrupled HIV screening rates and has identified new and out-of-care cases. Utilizing a continuous quality improvement approach has allowed it to become a catalyst to improve workflows for other gaps in University Medical Center of Southern Nevada's HIV care continuum process.

The workshop will engage participants to discuss what housing information is currently collected in their system and review what additional information it would be helpful to capture.

This interactive workshop will review updated data and discuss implementation challenges, facilitators, and best practices for routine screening/linkage in an emergency department with urban and rural challenges. Preliminary data from the initial two months (with more than 2,000 patients screened) include .7% HIV (including three acute infections), 7% Hepatitis C (HCV) and 5.2% syphilis.

Team members will share data around cure rates, reinfection, and engaging with marginalized patients, and discuss leveraging existing Ryan White infrastructure and resources for such efforts.
