Engaging the community and including faith institutions are key to improve access to care, treatment, and adherence. This session will highlight how three projects are working with implementing partners in the rural South.

Rapid has developed into a core feature of the Dorothy Mann Center HIV care continuum, assuring immediate linkage to expert HIV services, immediate initiation of therapy, and rapid viral suppression. Benefits are present for youth prevention services. Rapid access models are feasible and beneficial for youth HIV care and prevention.

Linkage Navigation Program provides an immediate connection to same-day medical care and medical case management services, promoting timely linkage to care and services, and tailored individualized care operationalizing initiatives such as Test N Treat and Test N PrEP with positive health outcomes for Ryan White clients.

This sub-study compared viral suppression between SPNS participants and non-participants at the Meharry site. The SPNS study enrolled women of color newly who were recently diagnosed with HIV or lost to care. There was no statistically significant difference in viral suppression after controlling for demographic and clinical factors.

Successes and lessons learned will be shared from three metropolitan areas on incorporating STI testing and treatment for prevention clients within an HIV medical home setting, along with how offering STI treatment impacts early identification service outcomes (e.g., HIV positivity rates, linkage to care referrals, and the rapid start of HIV treatment).

Establishing an opt-out HIV screening program in the emergency department has almost quadrupled HIV screening rates and has identified new and out-of-care cases. Utilizing a continuous quality improvement approach has allowed it to become a catalyst to improve workflows for other gaps in University Medical Center of Southern Nevada's HIV care continuum process.

A clinic-based substance abuse screening and treatment program is described. Using the Screening, Brief Intervention, and Referral to Treatment model, this provides annual proactive screening of alcohol/drug use, with a brief provider response and a follow-up motivational interviewing brief intervention, with treatment provided by an embedded provider.

Oklahoma has been classified by the Department of Health and Human Services as one of the seven states with a high rural HIV burden. Test-and-treat protocols are feasible within high-volume HIV clinics which serve rural and underserved communities to minimize the time to the first appointment to decrease time to viral load suppression.

This interactive workshop will review updated data and discuss implementation challenges, facilitators, and best practices for routine screening/linkage in an emergency department with urban and rural challenges. Preliminary data from the initial two months (with more than 2,000 patients screened) include .7% HIV (including three acute infections), 7% Hepatitis C (HCV) and 5.2% syphilis.

Ending the HIV Epidemic: A Plan for America provides an incredible opportunity for jurisdictions to assess their health systems, prevention activities, and linkage to care programs. It is important that health departments create innovative programs and examine how this initiative can be used to address viral hepatitis as well.

This presentation will discuss the rapid antiretroviral treatment (ART) programs implemented at the University Health System in the San Antonio metropolitan area and the Baltimore City Health Department clinics, as well as preliminary results post-implementation, lessons learned, and plans for future quality improvement in both programs.
