Eradicating Racism and Striving for Excellence in HIV Care (ERASE)
ERASE was developed to address the unique needs of Black MSM. Through an intensive case management intervention, peer case managers provide health education and wellness support, and connect clients to medical and behavioral healthcare. ERASE also offers a physical “safe space” for Black MSM to meet with a case manager, access medical services, or connect with peers. Louisiana Public Health Information Exchange
The Louisiana Public Health Information Exchange is a bidirectional exchange that connects hospital system electronic health records with state surveillance data. Providers use the exchange to identify and relink people with HIV who are out of care to clinical and supportive services. Viviendo Valiente
Viviendo Valiente aims to reduce ethnic disparities in HIV care and outcomes by providing culturally responsive services to the Latino/a community, specifically to people of Mexican descent. It is a multi-level intervention, featuring individual-, group-, and community-level activities, that links people to HIV care, offers HIV education and health literacy in group sessions, and promotes community-level testing for HIV and other sexually transmitted infections (STIs). Data to Care for People Coinfected with HIV and Hepatitis C Virus
The Michigan Department of Health and Human Services was one of seven health departments funded by Leveraging a Data to Care Approach to Cure Hepatitis C Virus (HCV) Within the RWHAP Part F SPNS initiative implemented from 2020–2022. With the support of the Yale University School of Medicine, which served as the Technical Assistance Provider, MDHHS matched RWHAP and HIV and HCV surveillance data, calculated HCV viral clearance cascades for coinfected populations, and worked with three RWHAP clinics to generate clinic-based lists of coinfected clients and conduct outreach and linkage to HCV treatment.Resource from the RWHAP Best Practices Compilation updated on 05/20/2024

LA Links (Louisiana Links)
LA Links is a combined data-to-care and client navigation approach that cross-references routinely collected HIV surveillance data with other secondary data sources to identify and locate people with HIV who are not in care, as well as those who are in care, but with high viral loads. Originally implemented in 2013 as part of the Care and Prevention in the United States Demonstration Project, LA Links improved linkage to care, reengagement in care, and viral suppression. LINK LA
LINK LA is a 12-session, 24-week peer navigation intervention for people with HIV who are scheduled to be released from incarceration. LINK LA peer navigators focus on behavioral changes that promote medication adherence and retention in care, while providing social support and facilitating communication with medical providers. Collecting and Using Data to Inform and Improve Information Dissemination
The TargetHIV website is the central hub for RWHAP technical assistance (TA), with an audience of 40,000 unique users a year. Of these, approximately 25 percent access resources from data-related TA providers. Peer Educators at Two Universities Leverage Technology to Cultivate Conversations About HIV Testing and Prevention
The results of training of peer educators to interview expert HIV providers in order to enhance student HIV knowledge and HIV testing in student health centers (and increased Vodcast viewing). Interview skills building was done by student health centers in collaboration with community partners including the Northeast/Caribbean AETC.

Assessing programmatic impact of movement between geographically clustered Part A service areas in Florida
RWHAP Part A programs in Florida are housed within county governments, not the state department of health. This prevents direct data sharing, leading to outdated out-of-care (OOC) lists. Migration of residents between counties complicates the management of OOC lists, creating a need for streamlined data exchange and matching.
