The Alexis Project

Bottom-Up Project
The Bottom-Up Project is a multi-organizational initiative focused on leveraging health information exchange data and peer navigation. Using real-time clinical data, in combination with linkage to HIV care and social services, the Bottom-Up Project locates and reengages people with HIV who are not currently in medical care and are not virally suppressed. Creating a Culture of Trauma-Informed Care
Lehigh Valley Health Network Comprehensive Health Services implemented a trauma-informed approach to care delivery, including training staff on how trauma can affect people’s health and how microaggressions in healthcare environments can potentially trigger trauma responses. Peer Engagement to Improve Linkage to Care and Improve Retention for Youth, Women, Infants and Children
University Health System’s peer engagement initiative employs peers, alongside Patient Navigators, to provide support and improve linkage and retention to care for youth, women, infants, and children. Group support activities and a chat-enabled website are two of the methods used to continually engage our population of focus.

Primero Yo: A Participant-Driven Group Empowering Latina Women with HIV to Find Their Voice
A culturally sensitive, participant-driven, Spanish-language group for Latina women with HIV promoted engagement in HIV services. Many participants had never previously engaged in support services. The increased participation resulted in multiple positive outcomes, including HIV viral load suppression, reduced stigma and isolation, engagement in mental health services, and increased community advocacy.
