As HIV elimination activities increase, jurisdictions can address HIV/Hepatitis C (HCV) co-infection by enhancing HCV screening and treatment data collection. This session will discuss NASTAD's work with North Carolina to incorporate HCV data in CAREWare and electronic health records (EHRs), as well as recommendations for integrating HCV services into the HIV care infrastructure.

The Lifia portal is a tool designed to manage client enrollment, eligibility and benefits, streamline and take an ADAP program to the next level.

University of Mississippi Medical Center utilized known barriers to care for postpartum women with HIV and leveraged that information to schedule individualized interactions with pregnant and postpartum women with HIV. This low-cost, low-effort initiative resulted in statistically significant improvements in both retention in care and viral suppression rates in postpartum women with HIV.

The Linkage to Care (LTC) Program at Denver Health/Denver Public Health is an innovative model using continuous quality improvement and community partners to close gaps in the HIV care continuum. This linkage model serves those seeking HIV prevention service as well as people with HIV seeking linkage and retention in care.

Establishing an opt-out HIV screening program in the emergency department has almost quadrupled HIV screening rates and has identified new and out-of-care cases. Utilizing a continuous quality improvement approach has allowed it to become a catalyst to improve workflows for other gaps in University Medical Center of Southern Nevada's HIV care continuum process.

Reaching viral suppression for the last 10% is challenging! Three different clinical sites across the state of Missouri identified a way to stratify unsuppressed patients – those that are consistently viremic and those that are unexpectedly viremic. This presentation will focus on quality improvement (QI) interventions for patients with persistent viremia.

Hennepin County Public Health (HCPH) matched a cohort of 5,500 Ryan White clients against county systems including Medicaid, other Minnesota public health care programs, food and cash support, emergency shelter and housing assistance, case management services, and the criminal justice system. HCPH is leveraging this data to reduce health disparities.

Community Health Worker (CHW) models are a strategy to reduce inequities in HIV. This presentation will describe an evaluation of 397 CHW program participants, with findings on the impact on barriers to care and clinical outcomes, as well as the relationship among unmet needs, CHW encounters, and clinical outcomes.

The New York City Department of Health and Mental Hygiene (Health Department) used a conceptual model of evidence-based practice implementation to outline a system for value-based payment for medical case management (MCM) services. The Health Department collaborated with key stakeholders, including MCM service providers, in the system’s design.

The Boston EMA built an innovative program to enhance provider capacity to incorporate clinical quality improvement (CQI) into its Ryan White service structure. The program provides a range of resources to support varied CQI needs across the EMA. Providers engaged in CQI have documented improved client outcomes including viral suppression and client satisfaction.

Since the revamping of its clinical quality management (CQM) program, it has continued to exceed HRSA benchmarks. This presentation will share unique and insightful experiences of implementing a CQM program.

Based on extensive assessments and ongoing input from clients and case managers, the collaborators have developed and deployed a ‘gamified’ version of the TAVIE-RED platform to engage, coach, and provide quality data that is are meaningful to providers. Speakers will present the platform and create an interactive process for participants.
