Clínica Bienestar
Clínica Bienestar (Spanish for “Wellness Clinic”) was developed to provide comprehensive, integrated HIV primary care services to Spanish-speaking and bilingual people of Puerto Rican ancestry, with HIV who inject drugs. Clínica Bienestar is a multilevel, multipronged intervention combining evidence-based practices in behavioral health and HIV medical care with a transnational approach to care. JumpstART
JumpstART launched in 2016 as part of New York State’s Ending the Epidemic initiative, changing the service delivery model of eight sexual health clinics to include an initial prescription of ART after an HIV diagnosis and prior to linkage to the community provider. Between November 2016 and September 2018, 60% of JumpstART clients received ART on the same day as diagnosis. Bottom-Up Project
The Bottom-Up Project is a multi-organizational initiative focused on leveraging health information exchange data and peer navigation. Using real-time clinical data, in combination with linkage to HIV care and social services, the Bottom-Up Project locates and reengages people with HIV who are not currently in medical care and are not virally suppressed. DISQ Office Hours: Gender-Affirming Data Collection
DISQ “Office Hours” are technical assistance opportunities that allow Ryan White HIV/AIDS Program (RWHAP) recipient and provider staff to talk with their peers in an informal setting about specific technical challenges and solutions. These hour-long calls include a short presentation by a RWHAP recipient or provider about their data management approach.

Coordination of Supportive Employment and Housing Services
The intervention integrated supportive employment services, housing services, and HIV care for clients receiving case management services and with unmet housing and employment needs. Patient-Centered Appointment Reminders
Allegheny Health Network implemented Patient-Centered Appointment Reminders over a five-month period to improve engagement in care for people with HIV. This intervention included text message reminders, a process for identifying and addressing barriers to care, home visits, and outreach to patients after missed appointments. POP-UP provides low-barrier comprehensive HIV primary care, substance use services, mental health services, and case management to people who are homeless and unstably housed with the goal of retaining clients in care and improving viral suppression. CoRECT
CoRECT was a data to care project to identify and reengage people with HIV who were newly out of care. It included a clinic and health department data reconciliation process to identify missed laboratory results or appointments and create the out-of-care list, case discussions via telephone to review the combined list, and field epidemiologist outreach to assist clients with making appointments, securing transportation, and arranging referrals. Stronger Together
Stronger Together uses counseling to increase engagement in and improve the quality of HIV care available for serodiscordant male couples. Through three in-person and additional “booster” sessions over an 18-month period, Stronger Together gave couples a space to work with an HIV professional to improve treatment adherence and keep both people healthy.
