This session will discuss how to reduce the percentage of HIV patients who are out of care. Participants will learn to use a multi-disciplinary team approach to identify patients who are truly out of care, pinpoint the most effective approach toward contacting patients to be brought back into HIV care, and maintain the patient’s HIV care.

These include apps, telemedicine, social media, and texting services that meet clients where they are and provide access to services. New HIV infections continue to rise among members of the Latinx community, especially youth. To reverse this increasing trend, it is important to recognize the social, economic, linguistic, and cultural barriers that hinder the prevention and treatment efforts for HIV when developing an outreach strategy.
