Implementing Evidence-Informed Interventions with Black MSM to Improve Access, Engagement/Reengagement, and Health Outcomes
This peer-led workshop will explore the implementation of evidence-informed interventions with black men who have sex with men (MSM) through Project CONNECT at AIDS Taskforce of Greater Cleveland, Tailored Motivational Interviewing at the University of Mississippi, and TXTXT at the SUNY-Downstate HEAT Program in Brooklyn. Positive Love: Improving Health Outcomes of Young, Black MSM with HIV in the Deep South
The presentation will cover the efficacy of +Love, a five-pronged intervention in New Orleans, involving a dedicated case manager, therapist, peer support, ride service, and text-based alert program that tracks clients' basic needs, mood, and provides medication reminders, in improving the healthcare and treatment satisfaction of young, black men who have sex with men (MSM) with HIV.

National Implementation of a Text-Messaging Intervention to Improve Antiretroviral Adherence among Youth with HIV
This workshop will explore two sites' (Brooklyn and Detroit) experience with the implementation of an evidence-informed intervention for black men who have sex with men (MSM) with HIV. The project seeks to improve HIV outcomes through the use of a culturally tailored intervention for Black MSM, the Text Messaging Intervention to Improve Antiretroviral Adherence among youth with HIV (TXTXT).

HCV Linkage, Screening, and Treatment among Co-infected Clients: Enhancing CAREWare and Electronic Health Records
As HIV elimination activities increase, jurisdictions can address HIV/Hepatitis C (HCV) co-infection by enhancing HCV screening and treatment data collection. This session will discuss NASTAD's work with North Carolina to incorporate HCV data in CAREWare and electronic health records (EHRs), as well as recommendations for integrating HCV services into the HIV care infrastructure.

Quality Improvement Strategies to Engage and Retain Postpartum Women with HIV
University of Mississippi Medical Center utilized known barriers to care for postpartum women with HIV and leveraged that information to schedule individualized interactions with pregnant and postpartum women with HIV. This low-cost, low-effort initiative resulted in statistically significant improvements in both retention in care and viral suppression rates in postpartum women with HIV.

Rapid Access for HIV Care and Prevention in a Center for Youth
Rapid has developed into a core feature of the Dorothy Mann Center HIV care continuum, assuring immediate linkage to expert HIV services, immediate initiation of therapy, and rapid viral suppression. Disseminated Evidence-Informed Initiative for Linkage of Women of Color to HIV Care: Virologic Outcome
This sub-study compared viral suppression between SPNS participants and non-participants at the Meharry site. The SPNS study enrolled women of color newly who were recently diagnosed with HIV or lost to care. There was no statistically significant difference in viral suppression after controlling for demographic and clinical factors.

Proactive Clinic-Based Substance Abuse Screening and Treatment: Helping to Advance in New Directions (Helping HAND)
A clinic-based substance abuse screening and treatment program is described. Using the Screening, Brief Intervention, and Referral to Treatment model, this provides annual proactive screening of alcohol/drug use, with a brief provider response and a follow-up motivational interviewing brief intervention, with treatment provided by an embedded provider.

Putting the PharmD in Adherence: An Evaluation of the Impact of a Clinical Pharmacist on Viral Suppression Rates
Through creative innovation, multiple approaches have been employed to assist patients with medication adherence. This presentation will look at the introduction of a clinical pharmacist in an urban, Ryan White-funded clinic, the impact on medication adherence, and the viral suppression and demographic trends observed over four years.
