UCLA Family AIDS Network, a Ryan White Part D recipient, will present its new Women and HIV bilingual education video series covering the topics of mental health and pregnancy. Rapid has developed into a core feature of the Dorothy Mann Center HIV care continuum, assuring immediate linkage to expert HIV services, immediate initiation of therapy, and rapid viral suppression. Benefits are present for youth prevention services. Rapid access models are feasible and beneficial for youth HIV care and prevention.

Establishing an opt-out HIV screening program in the emergency department has almost quadrupled HIV screening rates and has identified new and out-of-care cases. Utilizing a continuous quality improvement approach has allowed it to become a catalyst to improve workflows for other gaps in University Medical Center of Southern Nevada's HIV care continuum process.

A clinic-based substance abuse screening and treatment program is described. Using the Screening, Brief Intervention, and Referral to Treatment model, this provides annual proactive screening of alcohol/drug use, with a brief provider response and a follow-up motivational interviewing brief intervention, with treatment provided by an embedded provider.

This presentation will review the many roles a clinical pharmacist can play in improving patient care, such as providing medication education, conducting adherence counseling sessions, facilitating transitions in care, providing comprehensive medication chart reviews, and aiding with the formation of complex treatment regimens. Pharmacists' involvement in pre-exposure prophylaxis will also be reviewed.
