The New York City HIV Care Coordination Program is a structural intervention that combines multiple strategies, including multidisciplinary care coordination, patient navigation, and personalized health education to address client medical and social needs. Bottom-Up Project
The Bottom-Up Project is a multi-organizational initiative focused on leveraging health information exchange data and peer navigation. Using real-time clinical data, in combination with linkage to HIV care and social services, the Bottom-Up Project locates and reengages people with HIV who are not currently in medical care and are not virally suppressed. Performance Measures and the HIVQM
This webinar is open to all Ryan White HIV/AIDS Program recipients and subrecipients who are interested in using the HIV Quality Measures (HIVQM) Module. The HIVQM Module is a tool developed by HAB that allows recipients and subrecipients to enter their HAB performance measures and continually monitor their progress in improving health outcomes for people with HIV. HIVQM Module users can generate summary reports and compare their progress with other users regionally, nationally or by RWHAP Part.

INFINI-T
INFINI-T is a program designed to reach young transgender women of color with HIV and retain them in care through peer advocates who provided case management. These peer advocates facilitated social support sessions with the goal of alleviating the negative impacts of psychosocial factors like history of trauma, stigma, and discrimination.
