In this workshop, participants will gain resources and strategies for implementing community engagement programs, including training community health workers to be part of a health care team to enhance viral suppression and management of HIV in rural populations.

Community Health Worker (CHW) models are a strategy to reduce inequities in HIV. This presentation will describe an evaluation of 397 CHW program participants, with findings on the impact on barriers to care and clinical outcomes, as well as the relationship among unmet needs, CHW encounters, and clinical outcomes.

Project Consumer Access and Adherence to Care for HIV (CAATCH) is a Part D clinical quality initiative. Utilizing peers as partners, a series of health education sessions are delivered to make improvements on the HIV care continuum in engagement and viral suppression.

Unstable housing affects the ability of people with HIV to access, adhere to, and be retained in HIV medical care across the HIV care continuum. This presentation will examine adaptions made to housing and case management services in a Ryan White program to accommodate homeless and near-homeless people with HIV.

Part B, Part A, and the Minnesota Council for HIV/AIDS Care and Prevention collaborated to conduct a needs assessment of people with HIV. We facilitated meetings with various stakeholders to gain consensus on questions to include. It’s important to have strong project management, delegate responsibilities, and plan carefully.

Assessment of needs of people with HIV through broad-based community engagement is a tenet of the Ryan White planning process. The Baltimore EMA Planning Council developed a status-neutral survey targeting 1,100 persons. Modified Data Mapping supported community-based survey development by simplifying complex processes and creating transparent iterative processes of stakeholder engagement.

This workshop will describe how changes to an electronic health record (EHR) can capture programmatic outcomes data to evaluate program effectiveness, the benefits of developing custom tools within EHRs, how these EHR tools were developed, and how they have been integrated into program and evaluation work processes.

This session will present strategies to improve viral suppression in clients experiencing unstable housing and homelessness. This includes both a recipient serving homeless clients and a RWHAP AIDS Education and Training Center that trains clinicians on strategies to serve clients experiencing homelessness. The session concludes with an interactive participant Q&A period.
