As HIV elimination activities increase, jurisdictions can address HIV/Hepatitis C (HCV) co-infection by enhancing HCV screening and treatment data collection. This session will discuss NASTAD's work with North Carolina to incorporate HCV data in CAREWare and electronic health records (EHRs), as well as recommendations for integrating HCV services into the HIV care infrastructure.

The session will discuss lessons learned from the Rhode Island's path to the AIDS Drugs Assistance Program (ADAP) automation through the implementation of the Lifia Portal. The Lifia portal is a tool designed to manage client enrollment, eligibility and benefits, streamline and take an ADAP program to the next level.

In this workshop, two sites in HRSA's Improving Health Outcomes through the Coordination of Supportive Employment and Housing Services Initiative will describe how they work with their Part A Planning Councils and the Department of Housing and Urban Development's (HUD) Coordinated Entry System to identify and obtain permanent housing for people with HIV who are unstably housed.

Presenters will describe a New York City collaborative pilot project utilizing Regional Health Information Organizations (RHIOs) to locate lost-to-care people with HIV. Lost-to-care encounter alerts provide community-based organizations with actionable, real-time data to supplement their ‘classic' care engagement efforts. The model offers a potentially scalable, cost-effective strategy for patient re-engagement efforts on a population level.

This sub-study compared viral suppression between SPNS participants and non-participants at the Meharry site. The SPNS study enrolled women of color newly who were recently diagnosed with HIV or lost to care. There was no statistically significant difference in viral suppression after controlling for demographic and clinical factors.

This session will give information on the RWHAP SPNS Social Media Initiative (SMI), which used social media and digital technology to improve HIV outcomes for young people. It will provide cross-site outcomes from 10 demonstration sites. Three sites will discuss demonstration models. Resources for replicating the intervention models will be presented.

This session will help recipient and subrecipient sites to successfully connect their CAREWare and other electronic health information systems with external sources of data to report and monitor HIV care and treatment outcomes. This session will provide practical examples of successful data integration and highlight ongoing issues and costs in long-term management and maintenance.

People with HIV of low socioeconomic status may be at increased risk for food insecurity. This session will explore how to identify and address food insecurity in an HIV primary care setting. Current food assistance programs and related educational classes will be discussed, as well as relevant outcomes to HIV care.

As such, much of the innovation is happening at the state level. This panel will look at three different state-based programs focused on food and nutrition for people with HIV.

Care coordination providers in New York City have received comprehensive, multi-day training to develop and enhance skills for the effective delivery of patient navigation services. Training design supports navigation of healthcare and social services, coordination of logistics to support retention in care, adherence support, and viral suppression.
