Addressing STIs: Ask. Test. Treat. Repeat.
Nine sites implemented four evidence-based interventions, collectively known as Addressing STIs: Ask.Test.Treat.Repeat. The four intervention components are audio computer-assisted self-interview sexual history taking, patient self-collection of urogenital and extragenital site chlamydia/gonorrhea nucleic acid amplification test specimens, sexual and gender minority welcoming indicators, and provider training, with the overall goal to routinize STI screening, testing, and treatment in primary care. Addressing Opiate Use through Practice Transformation: Implementing Dashboard Reports to Improve Panel-Based Care
The U.S. opioid epidemic has significant impact on rates of overdose deaths among people with HIV. The aim of the project was to leverage the PDSA methodology to enhance a population-based, health information technology, intervention to facilitate the identification and panel management of high-risk clients with opioid use disorder.

Housing First as a Key Strategy to End the HIV Epidemic
Housing First is an effective, cost-efficient, evidence-based best practice that demonstrates improved health outcomes and care utilization for people experiencing homelessness, including people with HIV (PWH). Ending the HIV Epidemic in the U.S. (EHE) initiative is a unique opportunity for HRSA-funded Jurisdictions to support the uptake and implementation of the Housing First model in their communities to prevent new HIV diagnoses and engage people with HIV who are outside the current system of care.

Activities of jurisdictions and their partner clinics to implement Hepatitis C Virus (HCV) Data to Care project activities, based on their previous experience, data management infrastructure, ability to share data, and staffing resources.
The implementation manual summarizes approaches, lessons learned, and best practices for supporting the replication of efforts, and lists key resources for health department jurisdictions to start implementation.
