As HIV elimination activities increase, jurisdictions can address HIV/Hepatitis C (HCV) co-infection by enhancing HCV screening and treatment data collection. This session will discuss NASTAD's work with North Carolina to incorporate HCV data in CAREWare and electronic health records (EHRs), as well as recommendations for integrating HCV services into the HIV care infrastructure.

The session will discuss lessons learned from the Rhode Island's path to the AIDS Drugs Assistance Program (ADAP) automation through the implementation of the Lifia Portal. The Lifia portal is a tool designed to manage client enrollment, eligibility and benefits, streamline and take an ADAP program to the next level.

Panelists will describe collaborative prevention and support services provided to people with HIV and/or hepatitis from incarceration to re-entry. Services addressing social determinants of health are provided in corrections facilities and upon release. Panelists include the program director, staff from a correctional medical facility, and community-based organizations working inside and outside of correction facilities.

This session will provide practical examples of successful data integration and highlight ongoing issues and costs in long-term management and maintenance.

HRSA/HAB guidance on using RWHAP funds to provide services to eligible individuals in prisons, jails, and other correctional systems allows recipients to determine which RWHAP services to provide and for how long. This session will provide overviews of HAB policy and two recipients' programs serving people who are justice-involved.

The rate of HIV among people in US prisons is higher than the general population. This workshop will review an evidence-informed intervention to engage people with HIV in the jail setting. Three sites were funded in 2016 as part of Special Projects of National Significance Dissemination of Evidence-Informed Interventions initiative to implement an evidence-informed intervention designed to link people with HIV in jail to community HIV care using a transitional care coordinator. Presenters will discuss the barriers and facilitators related to linking to care and achieving and maintaining viral suppression post-release.

Maricopa County’s Part A Program used HRSA Policy 18-02, which defines allowable services for incarcerated individuals, to expand services in county jails. This session provides a clinical and administrative perspective on the processes, forms, and services that decreased time from jail-based diagnosis to treatment and viral load suppression.

In December 2019 the Iowa ADAP launched its Jail Assistance Program to extend HIV medication access to county jails. The ADAP partnered with a local jail to create a program that could be replicated across all of Iowa’s 99 counties. This presentation will focus on program development, utilization, and lessons learned.
