Community health care workers (CHWs) represent the voice of the community and play a vital role in linkage and retention to care. DC Health piloted three innovative care models to strategically ensure the community's voice is permanently embedded in HIV services.

In this workshop, two sites in HRSA's Improving Health Outcomes through the Coordination of Supportive Employment and Housing Services Initiative will describe how they work with their Part A Planning Councils and the Department of Housing and Urban Development's (HUD) Coordinated Entry System to identify and obtain permanent housing for people with HIV who are unstably housed.

Successes and lessons learned will be shared from three metropolitan areas on incorporating STI testing and treatment for prevention clients within an HIV medical home setting, along with how offering STI treatment impacts early identification service outcomes (e.g., HIV positivity rates, linkage to care referrals, and the rapid start of HIV treatment).

Workshop attendees will be introduced to the end+disparities ECHO Collaborative, the largest virtual community of practice of its kind. A panel of presenters will share their improvement interventions combating health disparities.

A clinic-based substance abuse screening and treatment program is described. Using the Screening, Brief Intervention, and Referral to Treatment model, this provides annual proactive screening of alcohol/drug use, with a brief provider response and a follow-up motivational interviewing brief intervention, with treatment provided by an embedded provider.

The New Jersey Behavioral Health and HIV Integration Project (NJ B-HIP) used an HIV and Behavioral Health Continuum and applied a kit of essential tools to achieve behavioral health and primary HIV care integration and improved outcomes. Specific frameworks, tools, and site-based examples will be shared, and cross-cutting issues will be discussed.

Cooper EIP Expanded Care Clinic in Camden, NJ has incorporated comprehensive Addictions Medicine (AM) care as part of its treatment model for people with HIV. A cohort of Substance Use Disorder (SUD) diagnosed patients displayed significant levels of VLS after enrolling in AM services.

It will explore culturally responsive leadership, transformational leadership, and cultural humility. Facilitators will demonstrate formal and informal leadership skills. Using data to identify disability-related needs of RWHAP provider agencies and clients to assess outcomes along the HIV Care Continuum, the New York HIV Planning Council developed with recipient support recommendations to better accommodate people with HIV with disabilities living in the New York EMA to improve service delivery, engagement, and viral suppression.
