The Alabama Quality Management Group (AQMG) is a consortium of quality leaders representing all Alabama RWHAP Part C and D clinics, over 80% of Alabama's HIV-diagnosed population. Data for Care Alabama (D4C-AL), a CDC-funded retention-in-care project, uses risk-stratification based on missed visits. It is currently being implemented at one site but will continue to be implemented over time.

Using data to identify disability-related needs of RWHAP provider agencies and clients to assess outcomes along the HIV Care Continuum, the New York HIV Planning Council developed with recipient support recommendations to better accommodate people with HIV with disabilities living in the New York EMA to improve service delivery, engagement, and viral suppression.

The HIV surveillance-based Agency-level Viral Suppression Report (AVSR) is intended to help RWHAP agencies understand viral suppression trends among their client population. Redesigning the AVSR to include viral suppression comparisons between additional client subgroups and vulnerable populations can expand agencies' capacity to identify health disparities and achieve equitable health outcomes.

Hennepin County Public Health (HCPH) matched a cohort of 5,500 Ryan White clients against county systems including Medicaid, other Minnesota public health care programs, food and cash support, emergency shelter and housing assistance, case management services, and the criminal justice system. HCPH is leveraging this data to reduce health disparities.

The New York City Health Department data-to-care model to eliminate Hepatitis C in people with HIV uses surveillance data to target health care practices for training and clinical practice facilitation, to identify patients for outreach and linkage to care, and to monitor progress toward elimination.

Using data from HIV surveillance and Ryan White data systems, this study describes characteristics of and disparities in HIV care outcomes among main HIV care providers in Hawaii. Less optimal viral suppression and retention in care were observed among those under care of providers who infrequently provided HIV medical care.
