Adapting and Implementing the Youth-Focused Case Management Model for the SPNS BMSM Initiative
Five sites participating in the SPNS Black Men who Have Sex with Men (BMSM) Initiative are implementing the Youth-focused Case Management model of care to support behavioral health services integration to improve HIV health outcomes. Lessons Learned From Linking ADAP Data with HIV Surveillance Data to Improve Health Outcomes in Massachusetts
HIV care measures for people with HIV enrolled in the Massachusetts AIDS Drug Assistance Program (ADAP) were compared to people with HIV not currently enrolled; results indicate that enrollees have higher frequencies of engagement and retention in care and viral suppression. Evaluation of the data provides insight that enhances programming and service delivery to reduce health disparities.

Positive Love: Improving Health Outcomes of Young, Black MSM with HIV in the Deep South
The presentation will cover the efficacy of +Love, a five-pronged intervention in New Orleans, involving a dedicated case manager, therapist, peer support, ride service, and text-based alert program that tracks clients' basic needs, mood, and provides medication reminders, in improving the healthcare and treatment satisfaction of young, black men who have sex with men (MSM) with HIV.

Utilizing Partnerships to Implement Insurance Benefit Management Services: Insurance Enrollment, Premium Payments and Co-Payments
Participants will learn about the presenters’ experience with implementing insurance benefit management services, including enrolling clients across Florida into insurance programs using a broker/assister hybrid model in collaboration with local Ryan White programs, developing an insurance benefits management technology infrastructure, and implementing medical copayment cards for Part A clients in Broward County.
