Adapting and Implementing the Youth-Focused Case Management Model for the SPNS BMSM Initiative

Implementing Evidence-Informed Interventions with Black MSM to Improve Access, Engagement/Reengagement, and Health Outcomes
This peer-led workshop will explore the implementation of evidence-informed interventions with black men who have sex with men (MSM) through Project CONNECT at AIDS Taskforce of Greater Cleveland, Tailored Motivational Interviewing at the University of Mississippi, and TXTXT at the SUNY-Downstate HEAT Program in Brooklyn. Lessons Learned From Linking ADAP Data with HIV Surveillance Data to Improve Health Outcomes in Massachusetts
HIV care measures for people with HIV enrolled in the Massachusetts AIDS Drug Assistance Program (ADAP) were compared to people with HIV not currently enrolled; results indicate that enrollees have higher frequencies of engagement and retention in care and viral suppression. Evaluation of the data provides insight that enhances programming and service delivery to reduce health disparities.

Positive Love: Improving Health Outcomes of Young, Black MSM with HIV in the Deep South
The presentation will cover the efficacy of +Love, a five-pronged intervention in New Orleans, involving a dedicated case manager, therapist, peer support, ride service, and text-based alert program that tracks clients' basic needs, mood, and provides medication reminders, in improving the healthcare and treatment satisfaction of young, black men who have sex with men (MSM) with HIV.

Developing Systems to Collect, Report, & Use Data to Optimize CHW Initiatives in the South (16076)
Through the Southern Initiative, CAI supported four Part A agencies in the South in integrating a Community Health Worker (CHW) model to address disparities in HIV outcomes. The session will describe the development of systems to collect, report, and use real-time data to track implementation progress and client outcomes. Results reveal promising client outcomes.

Health Information Exchange and Peer Navigation: A Model to Engage Vulnerable Populations
Presenters will describe a New York City collaborative pilot project utilizing Regional Health Information Organizations (RHIOs) to locate lost-to-care people with HIV. Lost-to-care encounter alerts provide community-based organizations with actionable, real-time data to supplement their ‘classic' care engagement efforts. The model offers a potentially scalable, cost-effective strategy for patient re-engagement efforts on a population level.

The Intersection of Substance Abuse, Depression, Violence and HIV: Implications for Ending the HIV Epidemic

Behavioral Health Co-location to Facilitate HIV and HCV Care in the Non-Metropolitan South
This session will discuss the implementation, outcomes, and future directions associated with the Health Services Center, Inc. Behavioral Health Co-location (BHC) Project. HSC is the sole provider of free HIV and Hepatitis C (HCV) services to a population facing multiple barriers to care (e.g., poverty, education and stigma) in a largely rural 14-county area of northeast Alabama.

The programs are Healthy Divas, and Transgender Women Engagement and Entry to Care Project (T.W.E.E.T.). Community Health Workers Improve Viral Load Suppression in Non-Treatment Adherent Populations to End the Epidemic
Community Health Workers (CHWs) were added to the care team to work with clients with poor treatment adherence and not viral load suppression (VLS). Clients benefited from CHW services that contributed to VLS in over 50% of the clients assigned to them. CHWs are vital in ending the epidemic in challenging client populations

From Stagnation to Celebration: An Overview of a Successful Community-Based Dental Partnership Program
This session is an overview of a Part F dental program's two-year journey to increase access to oral healthcare for people with HIV. The number of clients served, which was stagnant for several years, increased by 126% over the two-year initiative through increased collaboration and integration, dental case management, and innovative ways to increase capacity.
