This session will present strategies to improve viral suppression in clients experiencing unstable housing and homelessness. This includes both a recipient serving homeless clients and a RWHAP AIDS Education and Training Center that trains clinicians on strategies to serve clients experiencing homelessness. All RWHAP recipients and providers will experience an increase in their aging population and will need to respond. This session will give a firsthand perspective on aging with HIV, provide an overview of RWHAP aging data, summarize medical conditions and psychosocial needs facing aging clients, and review HRSA initiatives.

As HIV elimination activities increase, jurisdictions can address HIV/Hepatitis C (HCV) co-infection by enhancing HCV screening and treatment data collection. This session will discuss NASTAD's work with North Carolina to incorporate HCV data in CAREWare and electronic health records (EHRs), as well as recommendations for integrating HCV services into the HIV care infrastructure.

The session will discuss lessons learned from the Rhode Island's path to the AIDS Drugs Assistance Program (ADAP) automation through the implementation of the Lifia Portal. The Lifia portal is a tool designed to manage client enrollment, eligibility and benefits, streamline and take an ADAP program to the next level.

In this workshop, two sites in HRSA's Improving Health Outcomes through the Coordination of Supportive Employment and Housing Services Initiative will describe how they work with their Part A Planning Councils and the Department of Housing and Urban Development's (HUD) Coordinated Entry System to identify and obtain permanent housing for people with HIV who are unstably housed.

Boston Healthcare for the Homeless Program uses an innovative care model, designed from the margins, to meet the complex needs of people living at the intersections of HIV, homelessness, substance use disorder, and incarceration. Such models hold promise for closing HIV care and prevention equity gaps for this hyper-vulnerable group.

Boston Healthcare for the Homeless Program will share its innovative approach toward the micro-elimination of Hepatitis C (HCV) for homeless-experienced people with HIV. Team members will share data around cure rates, reinfection, and engaging with marginalized patients, and discuss leveraging existing Ryan White infrastructure and resources for such efforts.

People with HIV 50 years and older is a rapidly increasing population with unique needs. The presentation summarizes a population-based approach to assess those needs and to respond with training and other resources. Presenters will describe the implementation of the approach by the New York City EMA in partnership with the Northeast/Caribbean AETC.

This session will help recipient and subrecipient sites to successfully connect their CAREWare and other electronic health information systems with external sources of data to report and monitor HIV care and treatment outcomes. This session will provide practical examples of successful data integration and highlight ongoing issues and costs in long-term management and maintenance.

New York City EMA staff will present their work with older people with HIV, including planning efforts, focus group results, and first steps toward building best practices and developing a model of care for older people with HIV in urban settings.

This workshop will present the 6Ms, a simplified approach to geriatric principles for HIV care: Mobility, Mentation, Medications, Matters Most, Multicomplexity, and Modifiable. The role of the 6Ms in clinical care and identification of older adults at greatest need will be considered; clinical models of geriatric care with potential for widespread implementation will be presented.
