weCare Social Media Intervention
In weCare, a cyber health educator sent personalized messages through text, web-based apps, and Facebook to enrolled youth who identified as gay, bisexual, or other men who have sex with men (MSM), or transgender women, and who either had a new diagnosis of HIV or were not in care at the time of enrollment. Messages were personalized to each participant’s needs and were designed to support them as they navigated complicated health care systems as well as other challenges that affect care engagement (e.g., transportation, disclosure). The cyber health educator also moderated and posted information about health and well-being on an optional secret Facebook page that some participants chose to join. Participants were less likely to miss medical appointments and more likely to be virally suppressed after 12 months of the intervention.Resource from the RWHAP Best Practices Compilation updated on 01/07/2024

Postpartum Retention and Engagement Quality Improvement Initiative
The University of Mississippi Medical Center implemented a Postpartum Retention and Engagement Quality Improvement Initiative in 2017 to improve linkage to care, retention in care, and viral suppression among postpartum women with HIV. This intervention uses a combination of care coordination, printed materials, case management services, and improved collaboration and coordination between the Adult Special Care Clinic, which provides comprehensive HIV medical care, and a Perinatal HIV Program. A Third-Year Service Learning Rotation Program: Dental Students’ Perceptions of Community Engagement Experiences
Columbia University College of Dental Medicine has partnered with community partners to implement a dental care delivery system for PLWHA in NYC. A week-long Service Learning Rotation was integrated into the third-year dental student curriculum; data analysis from this pilot year will be used for program quality improvement and modification.

Impact of a Bilingual COVID-19 Vaccination Campaign in People Living with HIV
After language-specific outreach on getting a COVID-19 vaccination, project saw a high rate of vaccine uptake in Spanish-speaking patients (81%) compared to English-speaking patients (4%).

Housing First as a Key Strategy to End the HIV Epidemic
Housing First is an effective, cost-efficient, evidence-based best practice that demonstrates improved health outcomes and care utilization for people experiencing homelessness, including people with HIV (PWH). Ending the HIV Epidemic in the U.S. (EHE) initiative is a unique opportunity for HRSA-funded Jurisdictions to support the uptake and implementation of the Housing First model in their communities to prevent new HIV diagnoses and engage people with HIV who are outside the current system of care.
