HIV care measures for people with HIV enrolled in the Massachusetts AIDS Drug Assistance Program (ADAP) were compared to people with HIV not currently enrolled; results indicate that enrollees have higher frequencies of engagement and retention in care and viral suppression. Evaluation of the data provides insight that enhances programming and service delivery to reduce health disparities.

Telemedicine has emerged to provide medical care to those in rural settings. This model has not yet been utilized by Ryan White clinics at academic centers. We use a multidisciplinary approach to conduct visits with patients via live videoconferencing. There are multiple challenges, but there is potential for increasing access.

Using three HIV telehealth pilot projects as case studies, telehealth leaders from the Medical University of South Carolina—a HRSA-funded Telehealth Center of Excellence—will discuss how telehealth might be leveraged to improve access to HIV prevention and care. Telehealth technical assistance opportunities available through HRSA will also be highlighted.

This session will describe the development and implementation of a web-based electronic and administrative tracking system developed for use by clinic case managers. This electronic tracking tool allows case managers to easily view patient data with the overall goal of improving retention in care and adherence to medication.

Georgia’s telehealth/telemedicine and Project ECHO programs aim to identify and tackle barriers faced by urban and rural environments. Telehealth offers patients a virtual way of meeting with their care providers, while also improving patient engagement and care plan adherence. Project ECHO is aimed at workforce development and capacity building.

Caracole, an AIDS Service Organization, uses three interconnected approaches to improve retention in HIV care: housing first, harm reduction, and motivational interviewing. Clients in permanent supportive housing had high rates of viral suppression, exceeding Caracole's goal of 75%.
