The STAR Program, a Ryan White Part C program, initiated a 30-minute, weekly televised TV health magazine that engages the community across multiple social media and television platforms with a focus on health particularly HIV treatment and Prevention, reaching young adults. The presentation will cover the efficacy of +Love, a five-pronged intervention in New Orleans, involving a dedicated case manager, therapist, peer support, ride service, and text-based alert program that tracks clients' basic needs, mood, and provides medication reminders, in improving the healthcare and treatment satisfaction of young, black men who have sex with men (MSM) with HIV.

This workshop will explore two sites' (Brooklyn and Detroit) experience with the implementation of an evidence-informed intervention for black men who have sex with men (MSM) with HIV. The project seeks to improve HIV outcomes through the use of a culturally tailored intervention for Black MSM, the Text Messaging Intervention to Improve Antiretroviral Adherence among youth with HIV (TXTXT).

In this workshop, two sites in HRSA's Improving Health Outcomes through the Coordination of Supportive Employment and Housing Services Initiative will describe how they work with their Part A Planning Councils and the Department of Housing and Urban Development's (HUD) Coordinated Entry System to identify and obtain permanent housing for people with HIV who are unstably housed.

The panel will discuss experiences working with Ryan White and housing data, including the development of expanded housing definitions and findings from research. Workshop attendees will be introduced to the end+disparities ECHO Collaborative, the largest virtual community of practice of its kind. Using data to identify disability-related needs of RWHAP provider agencies and clients to assess outcomes along the HIV Care Continuum, the New York HIV Planning Council developed with recipient support recommendations to better accommodate people with HIV with disabilities living in the New York EMA to improve service delivery, engagement, and viral suppression.

Oklahoma has been classified by the Department of Health and Human Services as one of the seven states with a high rural HIV burden. Test-and-treat protocols are feasible within high-volume HIV clinics which serve rural and underserved communities to minimize the time to the first appointment to decrease time to viral load suppression.

This session will give information on the RWHAP SPNS Social Media Initiative (SMI), which used social media and digital technology to improve HIV outcomes for young people. It will provide cross-site outcomes from 10 demonstration sites. Three sites will discuss demonstration models. Resources for replicating the intervention models will be presented.

Department of Health efforts to rapidly engage diverse community members and the regional integrated care and prevention planning commission for the District and neighboring jurisdictions to inform accelerated strategies and tasks for the DC 90/90/90/50 Plan 2.0 to End the HIV Epidemic (EHE).

Transgender women, especially those of color, have substantially lower rates of viral suppression than people with HIV in general. The New York City HIV Planning Council collaborated with the community to develop guiding recommendations that drove the development of the jurisdiction's first psycho-social service directive specifically for people of transgender, intersex, non-binary, and/or non-conforming experience.
