Innovative Approach to MAI: A Data-Driven Model to Improve Health Outcomes among Massachusetts ADAP Participants

The Power of HIV Treatment and Prevention Content across Multiple Platforms in Promoting Community Engagement
The STAR Program, a Ryan White Part C program, initiated a 30-minute, weekly televised TV health magazine that engages the community across multiple social media and television platforms with a focus on health particularly HIV treatment and Prevention, reaching young adults. Positive Love: Improving Health Outcomes of Young, Black MSM with HIV in the Deep South
The presentation will cover the efficacy of +Love, a five-pronged intervention in New Orleans, involving a dedicated case manager, therapist, peer support, ride service, and text-based alert program that tracks clients' basic needs, mood, and provides medication reminders, in improving the healthcare and treatment satisfaction of young, black men who have sex with men (MSM) with HIV.

National Implementation of a Text-Messaging Intervention to Improve Antiretroviral Adherence among Youth with HIV
This workshop will explore two sites' (Brooklyn and Detroit) experience with the implementation of an evidence-informed intervention for black men who have sex with men (MSM) with HIV. The project seeks to improve HIV outcomes through the use of a culturally tailored intervention for Black MSM, the Text Messaging Intervention to Improve Antiretroviral Adherence among youth with HIV (TXTXT).

HCV Linkage, Screening, and Treatment among Co-infected Clients: Enhancing CAREWare and Electronic Health Records
As HIV elimination activities increase, jurisdictions can address HIV/Hepatitis C (HCV) co-infection by enhancing HCV screening and treatment data collection. This session will discuss NASTAD's work with North Carolina to incorporate HCV data in CAREWare and electronic health records (EHRs), as well as recommendations for integrating HCV services into the HIV care infrastructure.

RWHAP SPNS Initiative: Using Social and Digital Media to Improve HIV Care in Young People
This session will give information on the RWHAP SPNS Social Media Initiative (SMI), which used social media and digital technology to improve HIV outcomes for young people. It will provide cross-site outcomes from 10 demonstration sites. Three sites will discuss demonstration models. Resources for replicating the intervention models will be presented.

Managing Multiple Data Sources and Overcoming Interoperability Barriers for HIV Care
This session will help recipient and subrecipient sites to successfully connect their CAREWare and other electronic health information systems with external sources of data to report and monitor HIV care and treatment outcomes. Housing First to Treat and Prevent HIV
Caracole, an AIDS Service Organization, uses three interconnected approaches to improve retention in HIV care: housing first, harm reduction, and motivational interviewing. Clients in permanent supportive housing had high rates of viral suppression, exceeding Caracole's goal of 75%.
