Adapting and Implementing the Youth-Focused Case Management Model for the SPNS BMSM Initiative

Lessons Learned From Linking ADAP Data with HIV Surveillance Data to Improve Health Outcomes in Massachusetts
HIV care measures for people with HIV enrolled in the Massachusetts AIDS Drug Assistance Program (ADAP) were compared to people with HIV not currently enrolled; results indicate that enrollees have higher frequencies of engagement and retention in care and viral suppression. Evaluation of the data provides insight that enhances programming and service delivery to reduce health disparities.

Innovative, Integrated Care: Two Unique Deliveries of Telepsychiatry Services
Telepsychiatry has proven to provide better access and higher-quality care to patients who need psychiatric care as well as for those who have varying circumstances that make it difficult to engage in this service. Vivent Health has successfully integrated telepsychiatry within its medical home model. With two different service delivery methods, this presentation will look at the benefits of telepsychiatry for people with HIV, as well as the unique delivery methods Vivent Health provides in Wisconsin and Colorado.

Support Services Trifecta: Creative Utilization of Transportation, Housing, and Food

Incorporating Telemedicine into Care at a Large Urban HIV Clinic
Access to telemedicine visits in a multidisciplinary urban HIV clinic allows for improved access to care for patients who have fallen out of care or those who have difficulty attending their appointments due to stigma, multiple comorbidities, or barriers related to work, transportation, or childcare.
