Reaching and engaging rural populations in care and treatment is critical to ending the HIV epidemic. Engaging the community and including faith institutions are key to improve access to care, treatment, and adherence. This session will highlight how three projects are working with implementing partners in the rural South.

University of Mississippi Medical Center utilized known barriers to care for postpartum women with HIV and leveraged that information to schedule individualized interactions with pregnant and postpartum women with HIV. This low-cost, low-effort initiative resulted in statistically significant improvements in both retention in care and viral suppression rates in postpartum women with HIV.

Rapid has developed into a core feature of the Dorothy Mann Center HIV care continuum, assuring immediate linkage to expert HIV services, immediate initiation of therapy, and rapid viral suppression. Benefits are present for youth prevention services. Rapid access models are feasible and beneficial for youth HIV care and prevention.

Presenters will describe a New York City collaborative pilot project utilizing Regional Health Information Organizations (RHIOs) to locate lost-to-care people with HIV. Lost-to-care encounter alerts provide community-based organizations with actionable, real-time data to supplement their ‘classic' care engagement efforts. The model offers a potentially scalable, cost-effective strategy for patient re-engagement efforts on a population level.

This panel will provide an opportunity to hear federal updates on Hepatitis C surveillance and learn about a data-to-care approach and overcoming surveillance data gaps.

A clinic-based substance abuse screening and treatment program is described. Using the Screening, Brief Intervention, and Referral to Treatment model, this provides annual proactive screening of alcohol/drug use, with a brief provider response and a follow-up motivational interviewing brief intervention, with treatment provided by an embedded provider.

The New Jersey Behavioral Health and HIV Integration Project (NJ B-HIP) used an HIV and Behavioral Health Continuum and applied a kit of essential tools to achieve behavioral health and primary HIV care integration and improved outcomes. Specific frameworks, tools, and site-based examples will be shared, and cross-cutting issues will be discussed.

Oklahoma has been classified by the Department of Health and Human Services as one of the seven states with a high rural HIV burden. Test-and-treat protocols are feasible within high-volume HIV clinics which serve rural and underserved communities to minimize the time to the first appointment to decrease time to viral load suppression.

After childbirth, women with HIV are more likely to fall out of care, leading to higher morbidity, risk of transmitting HIV to intimate partners, and subsequent pregnancies. Psychiatric disorders and other barriers are contributors to loss of follow-up. We present the successes and opportunities to improve health outcomes for postpartum women with HIV.

This workshop will highlight the AIDS Education Training Centers' (AETC) Core Training activities from 2016 through 2019, focusing on types of providers reached, priority training topics, and training modalities. Two AETCs will share experiences engaging rural jurisdictions with low-volume HIV providers and approaches for reaching providers in the Ending the HIV Epidemic initiative jurisdictions.

This session highlights specific HRSA and CDC resources used to reduce HIV-related stigma in rural areas and a recipient's best practices to reduce stigma in the healthcare setting to improve clients' health outcomes and quality of life. No-show rate is a strong and independent predictor of patients falling out of care at the partnership that cares for 1,700 people with HIV in Philadelphia. A pilot study was created to address every no-show event in an effort to prevent patients from falling out of care.
