Selected Sections

The following five sections are examples of strong Assessing Needs, Gaps, and Barriers sections developed by jurisdictions. Each document contains a cover sheet with the jurisdiction information and why the section was identified as exemplary.

Selection Criteria

Exemplary Assessing Needs, Gaps, and Barriers sections met the following criteria (based on the Integrated HIV Prevention and Care Plan Guidance):

  • Includes description of process used to identify HIV prevention and service needs of those at risk and people with HIV
  • Demonstrates engagement of those at risk and people with HIV in planning
  • Clear and robust description of service needs
  • Clear and robust description of service gaps
  • Clear and robust description of barriers (social, policy, health department, program, service provider, and client barriers).

Plans with particularly strong aspects were chosen to be featured.

Please note. Those featured do not include all assessing needs, gaps, and barriers sections of Integrated HIV Prevention and Care Plans that are exemplary.
