Workshop attendees will be introduced to the end+disparities ECHO Collaborative, the largest virtual community of practice of its kind. Transgender women, especially those of color, have substantially lower rates of viral suppression than people with HIV in general. The New York City HIV Planning Council collaborated with the community to develop guiding recommendations that drove the development of the jurisdiction's first psycho-social service directive specifically for people of transgender, intersex, non-binary, and/or non-conforming experience.

This workshop will explore the implementation of peer-led, evidence-informed interventions for transgender women with HIV. The programs are Healthy Divas, and Transgender Women Engagement and Entry to Care Project (T.W.E.E.T.). Attendees will learn the core elements of these interventions and how they improve engagement in care, treatment adherence, and viral suppression in Alabama and Michigan.

Telemedicine has emerged to provide medical care to those in rural settings. This model has not yet been utilized by Ryan White clinics at academic centers. We use a multidisciplinary approach to conduct visits with patients via live videoconferencing. There are multiple challenges, but there is potential for increasing access.

Using three HIV telehealth pilot projects as case studies, telehealth leaders from the Medical University of South Carolina—a HRSA-funded Telehealth Center of Excellence—will discuss how telehealth might be leveraged to improve access to HIV prevention and care. Telehealth technical assistance opportunities available through HRSA will also be highlighted.

Prior analyses among transgender women receiving Ryan White Part A services in New York City have found evidence of disparities in HIV outcomes. This presentation will focus on viral suppression and the New York Eligible Metropolitan Area’s efforts to address care continuum outcome disparities affecting clients of transgender experience.

Georgia’s telehealth/telemedicine and Project ECHO programs aim to identify and tackle barriers faced by urban and rural environments. Telehealth offers patients a virtual way of meeting with their care providers, while also improving patient engagement and care plan adherence. Project ECHO is aimed at workforce development and capacity building.
