Services addressing social determinants of health are provided in corrections facilities and upon release. Panelists include the program director, staff from a correctional medical facility, and community-based organizations working inside and outside of correction facilities.

This session will provide overviews of HAB policy and two recipients' programs serving people who are justice-involved.
