As part of the SPNS Social Media Initiative (SMI), three Ryan White recipients describe their experiences with text messaging, both automated and real-time, to increase engagement in care and the number of medical appointments kept, and improve viral suppression rates with an emphasis on engaging young men who have sex with men and trans women of color.

Managing the needs of people with HIV can be difficult and intensive. The importance of caring for oneself is often overlooked by program staff and can threaten their well-being. Staff who provide non-medical case management and assistance in finding employment and housing were interviewed to determine key areas of concern.

Despite systemwide efforts to increase capacity for quality improvement (QI) there remains challenges in bridging the gap between quality improvement concepts, best clinical practices, and client barriers to quality care. To address this gap, the Broward EMA utilized a learning collaborative approach to improve providers’ capacity to conduct QI.

HRSA/HAB Policy Clarification Notice 15-02 provides guidance on Clinical Quality Management (CQM) program infrastructure components. The content will focus on planning your CQM program, developing your work plan, and building the CQM committee. This session is intended for people with less than two years of experience in CQM.

Recipients will share how to meet the HRSA HAB policy clarification notice (PCN) 15-02 guidance regarding CQM program activity identification for their subrecipients and how to confirm subrecipients have the capacity and resources to contribute to the recipient’s CQM program.

Prism Health North Texas will share the challenges and successes of integrating non-medical case management workflow processes into an integrated electronic health record system. The presenter will discuss workflow processes, assessments that allow non-medical case managers to gather required information and determine outcomes and identify patients appropriate for program graduation.

Part B, Part A, and the Minnesota Council for HIV/AIDS Care and Prevention collaborated to conduct a needs assessment of people with HIV. We facilitated meetings with various stakeholders to gain consensus on questions to include. It’s important to have strong project management, delegate responsibilities, and plan carefully.

Assessment of needs of people with HIV through broad-based community engagement is a tenet of the Ryan White planning process. The Baltimore EMA Planning Council developed a status-neutral survey targeting 1,100 persons. Modified Data Mapping supported community-based survey development by simplifying complex processes and creating transparent iterative processes of stakeholder engagement.
