Telepsychiatry has proven to provide better access and higher-quality care to patients who need psychiatric care as well as for those who have varying circumstances that make it difficult to engage in this service. Vivent Health has successfully integrated telepsychiatry within its medical home model. With two different service delivery methods, this presentation will look at the benefits of telepsychiatry for people with HIV, as well as the unique delivery methods Vivent Health provides in Wisconsin and Colorado.

A case study of one Part B subrecipient improved linkage and retention rates through the innovative use of medical transportation, housing services, and food bank and home-delivered meals. The presentation will share lessons learned and propose strategies to replicate these services elsewhere.

Despite systemwide efforts to increase capacity for quality improvement (QI) there remains challenges in bridging the gap between quality improvement concepts, best clinical practices, and client barriers to quality care. The content will focus on planning your CQM program, developing your work plan, and building the CQM committee. This session is intended for people with less than two years of experience in CQM.

Recipients determine how subrecipients will contribute to the recipients’ Clinical Quality Management (CQM) program. Recipients will share how to meet the HRSA HAB policy clarification notice (PCN) 15-02 guidance regarding CQM program activity identification for their subrecipients and how to confirm subrecipients have the capacity and resources to contribute to the recipient’s CQM program.

Part B, Part A, and the Minnesota Council for HIV/AIDS Care and Prevention collaborated to conduct a needs assessment of people with HIV. We facilitated meetings with various stakeholders to gain consensus on questions to include. It’s important to have strong project management, delegate responsibilities, and plan carefully.

Assessment of needs of people with HIV through broad-based community engagement is a tenet of the Ryan White planning process. The Baltimore EMA Planning Council developed a status-neutral survey targeting 1,100 persons. Modified Data Mapping supported community-based survey development by simplifying complex processes and creating transparent iterative processes of stakeholder engagement.

This informative, interactive workshop will inform participants of the basic tenets of developing and revising a clinical quality management (CQM) plan and associated work plan, based on operational capacity and CQM program planned activities. Using their own CQM plans, participants will assess and revise each section with the guidance of subject matter experts.

This session will present strategies to improve viral suppression in clients experiencing unstable housing and homelessness. This includes both a recipient serving homeless clients and a RWHAP AIDS Education and Training Center that trains clinicians on strategies to serve clients experiencing homelessness. The session concludes with an interactive participant Q&A period.
