The session will discuss lessons learned from the Rhode Island's path to the AIDS Drugs Assistance Program (ADAP) automation through the implementation of the Lifia Portal. The Lifia portal is a tool designed to manage client enrollment, eligibility and benefits, streamline and take an ADAP program to the next level.

The presentation will include an overview of the transition to self-attestation and how organizational change supported a quality-driven approach.

This presentation will review how the Michigan Department of Health and Human Services Chronic Disease Division collaborated with the HIV/STI division and local AIDS service organizations to develop innovative, client-centered tobacco dependence treatment programs to address the high tobacco use rates among people with HIV.

Transgender women, especially those of color, have substantially lower rates of viral suppression than people with HIV in general. The New York City HIV Planning Council collaborated with the community to develop guiding recommendations that drove the development of the jurisdiction's first psycho-social service directive specifically for people of transgender, intersex, non-binary, and/or non-conforming experience.

This workshop will explore the implementation of peer-led, evidence-informed interventions for transgender women with HIV. The programs are Healthy Divas, and Transgender Women Engagement and Entry to Care Project (T.W.E.E.T.). Attendees will learn the core elements of these interventions and how they improve engagement in care, treatment adherence, and viral suppression in Alabama and Michigan.

Presenters will describe a medical community collaboration to resolve care gaps and improve patient health outcomes through the co-location of health home care management by a community-based organization (CBO) within medical clinics. This presentation will focus on viral suppression and the New York Eligible Metropolitan Area’s efforts to address care continuum outcome disparities affecting clients of transgender experience.

This report highlights projects found to offer highly effective (and replicable) approaches to integrating buprenorphine-based medication-assisted treatment into HIV/AIDS primary care. Chapters outline the processes programs need to put in place, including meeting DATA 2000 regulatory requirements, achieving institutional buy-in, implementing staff training requirements, and evaluating processes and outcomes.

The Northeast/Caribbean AIDS Education and Training Center HIV Health Information Exchange (HIE) Initiative aims to empower HIV care networks to utilize the potential of emerging data exchange models toward improving HIV outcomes and ending the epidemic. The program convenes and educates stakeholders, supporting efforts toward collaborative planning.
