The presentation will focus on the specifics of delivering brief mental health in-clinic interventions and the development and implementation of youth peer support groups within the pediatric HIV program in the ‘hot spot' of the national HIV epidemic, Washington, D.C.

Community health care workers (CHWs) represent the voice of the community and play a vital role in linkage and retention to care. DC Health piloted three innovative care models to strategically ensure the community's voice is permanently embedded in HIV services.

Through the Southern Initiative, CAI supported four Part A agencies in the South in integrating a Community Health Worker (CHW) model to address disparities in HIV outcomes. The session will describe the development of systems to collect, report, and use real-time data to track implementation progress and client outcomes. Results reveal promising client outcomes.

Presenters will describe a New York City collaborative pilot project utilizing Regional Health Information Organizations (RHIOs) to locate lost-to-care people with HIV. Lost-to-care encounter alerts provide community-based organizations with actionable, real-time data to supplement their ‘classic' care engagement efforts. The model offers a potentially scalable, cost-effective strategy for patient re-engagement efforts on a population level.

The Alabama Quality Management Group (AQMG) is a consortium of quality leaders representing all Alabama RWHAP Part C and D clinics, over 80% of Alabama's HIV-diagnosed population. Data for Care Alabama (D4C-AL), a CDC-funded retention-in-care project, uses risk-stratification based on missed visits. It is currently being implemented at one site but will continue to be implemented over time.

Using data to identify disability-related needs of RWHAP provider agencies and clients to assess outcomes along the HIV Care Continuum, the New York HIV Planning Council developed with recipient support recommendations to better accommodate people with HIV with disabilities living in the New York EMA to improve service delivery, engagement, and viral suppression.

This presentation focuses on peer-based training for engaging consumer voices in HIV providers' quality improvement efforts. Community-based, peer-led efforts impact HIV care program effectiveness. Peer-led training concentrating on client advocacy and involvement in quality improvement is another method for promoting intentional collaboration between HIV service programs and people with HIV.

This workshop will explore the implementation of peer-led, evidence-informed interventions for transgender women with HIV. The programs are Healthy Divas, and Transgender Women Engagement and Entry to Care Project (T.W.E.E.T.). Attendees will learn the core elements of these interventions and how they improve engagement in care, treatment adherence, and viral suppression in Alabama and Michigan.

The session will discuss how Arizona is implementing the Data to Care program by using REDCap and HIV surveillance data (from eHARS, CAREWare, and other outside databases) to successfully prioritize and disposition persons with HIV who are no longer in care.

Community Health Workers (CHWs) were added to the care team to work with clients with poor treatment adherence and not viral load suppression (VLS). Clients benefited from CHW services that contributed to VLS in over 50% of the clients assigned to them. CHWs are vital in ending the epidemic in challenging client populations

Data integration projects are more successful when community voice is involved, and projects benefit from multidisciplinary team buy-in. Hear from the city of Hartford, Connecticut, and the city of Kansas City, Missouri, each of whom launched cross-sectoral workgroups to shepherd various types of data integration planning, implementation, and sustainability.

This session is an overview of a Part F dental program's two-year journey to increase access to oral healthcare for people with HIV. The number of clients served, which was stagnant for several years, increased by 126% over the two-year initiative through increased collaboration and integration, dental case management, and innovative ways to increase capacity.

Massachusetts' statewide HIV Dental Services program is a unique collaboration across Ryan White Parts A and B to ensure all eligible residents with HIV in need of oral health services receive them. Data show that access to the program is associated with improved care continuum outcomes.
