Driven by Community: A NYC Directive to Improve Health Outcomes for People of Transgender Experience
Transgender women, especially those of color, have substantially lower rates of viral suppression than people with HIV in general. The New York City HIV Planning Council collaborated with the community to develop guiding recommendations that drove the development of the jurisdiction's first psycho-social service directive specifically for people of transgender, intersex, non-binary, and/or non-conforming experience.

Implementing Evidence-Informed Interventions with Transgender Women to Improve Access, Engagement, and Health Outcomes
This workshop will explore the implementation of peer-led, evidence-informed interventions for transgender women with HIV. The programs are Healthy Divas, and Transgender Women Engagement and Entry to Care Project (T.W.E.E.T.). Attendees will learn the core elements of these interventions and how they improve engagement in care, treatment adherence, and viral suppression in Alabama and Michigan.

Expanding Oral Healthcare Access Through an ADAP-Designed Dental Program
In 2019, the Louisiana AIDS Drug Assistance Program (ADAP) partnered with a leading private dental insurer to design a group dental plan modeled after an employer-sponsored plan, specifically tailored to the needs of people with HIV. Both utilization and client satisfaction rose significantly following the adoption of the plan.

Presenters will describe a medical community collaboration to resolve care gaps and improve patient health outcomes through the co-location of health home care management by a community-based organization (CBO) within medical clinics. Prior analyses among transgender women receiving Ryan White Part A services in New York City have found evidence of disparities in HIV outcomes. This presentation will focus on viral suppression and the New York Eligible Metropolitan Area’s efforts to address care continuum outcome disparities affecting clients of transgender experience.
