Three case studies about success strategies needed for different regional recipients: the Iowa Department of Public Health and the Part A programs of Dallas, Texas, and Paterson, N.J. Each program will detail how it used an online evidenced-based approach to improve its response to the epidemic.

Transgender women, especially those of color, have substantially lower rates of viral suppression than people with HIV in general. The New York City HIV Planning Council collaborated with the community to develop guiding recommendations that drove the development of the jurisdiction's first psycho-social service directive specifically for people of transgender, intersex, non-binary, and/or non-conforming experience.

This workshop will explore the implementation of peer-led, evidence-informed interventions for transgender women with HIV. The programs are Healthy Divas, and Transgender Women Engagement and Entry to Care Project (T.W.E.E.T.). Attendees will learn the core elements of these interventions and how they improve engagement in care, treatment adherence, and viral suppression in Alabama and Michigan.

Presentation of a mixed-methods needs assessment to inform a multi-site evaluation plan for a HRSA Special Projects of National Significance (SPNS) project to improve bacterial STI screening, testing, and treatment among people with or at risk for HIV will be presented, including findings, selected evidence-based interventions, and a proposed multi-site evaluation plan.

Presenters will describe a medical community collaboration to resolve care gaps and improve patient health outcomes through the co-location of health home care management by a community-based organization (CBO) within medical clinics.
